Dr Matea Deliu is an academic GP and the Associate Medical Director for One Health Lewisham. Dr Deliu’s special interest is in digital health and the use of AI in primary care and respiratory medicine. She currently oversees the NHS@Home service in Lewisham which is a large pioneering digitally first virtual ward. As a GP, Dr. Deliu works with Guy’s and St. Thomas’s @Home service.

Chief Executive Officer, National Academy for Social Prescribing
Charlotte is CEO of the National Academy for Social Prescribing, a charity funded by government to support the development and expansion of social prescribing in healthcare and the wider community. This includes development of new investment models, driving improvements in evidence and evaluation and supporting systems to utilise social prescribing to address critical challenges and embed personalised, holistic care. The Academy also works internationally, leading a collaborative of partners from 30 countries for knowledge exchange and global development. She has held leadership roles in the charity sector for 15 years and worked closely with many Primary Care Networks and NHS colleagues to shape, deliver and scale social prescribing. This has included a focus on addressing health inequalities and new commissioning models for voluntary sector services. @NASPTweets

Dr Shanker Vijay is London’s GP Lead for Digital Transformation (Primary Care) at NHS England (London). He is also a Digital/IT lead in North West London ICB.
Shanker enjoys simple innovations to tackle problems facing both patients and staff. In his London role, he is presently particularly involved in Automation/AI, Pharmacy First, Online registration, Online consultations/Digital Tools and NHS App. Shanker has been a GP since 2004 and works clinically in North West London. He embraces online patient feedback and is currently rated the number 1 GP on iWantGreatCare. He was a shortlisted finalist for an EHI Award in 2011 for the “Best Use of Social Media in Healthcare”. Consultant Geriatrician, Croydon Health Services, South West London ICB, NHS England
Towhid is a Consultant Geriatrician and leading the Front Door Frailty service in Croydon Hospital. He is a GIRFT Virtual Ward Clinical Lead and advises on policy around older people and frailty at NHS England and in South West London ICB as a Clinical Lead.

The hotel is just a 3-minute walk from Hammersmith Underground Station and less than 20-minutes travel from Central London or Heathrow.
